About the Provider
Kifah Integrative Health was founded by a board-certified Family Nurse Practitioner with extensive experience caring for adults and older adults in primary care, internal medicine, skilled nursing, rehabilitation, cardiac care, and home health settings.
Our goal is to make quality healthcare more accessible for patients who have difficulty traveling to a traditional medical office due to age, chronic medical conditions, mobility limitations, or other health concerns.
We believe healthcare should be personal, compassionate, and convenient. By providing medical care in the patient's home, we are able to better understand each patient's individual needs, medications, home environment, and support system while working closely with families, specialists, pharmacies, home health agencies, therapists, and other members of the healthcare team.

In-Home Primary Care Visits
Comprehensive medical visits provided in the comfort of the patient's home for eligible patients who have difficulty accessing traditional office-based care.
Chronic Disease Management
Ongoing evaluation and management of chronic medical conditions such as diabetes, high blood pressure, heart failure, COPD, kidney disease, and more.
Medication Management
Medication review, reconciliation, refills when clinically appropriate, monitoring for medication interactions and side effects, and coordination with pharmacies.
Post-Hospital & Rehabilitation Follow-Up
Follow-up care after hospitalization, emergency room visits, skilled nursing facility stays, or rehabilitation to help patients safely transition back home.
Home Health Care Coordination & Oversight
Evaluation and coordination of medically necessary home health services when appropriate, including skilled nursing, physical therapy, occupational therapy, and other services.
Laboratory & Diagnostic Coordination
Ordering and coordinating medically necessary laboratory testing, imaging, and diagnostic services, including mobile services when available and appropriate.
Specialist Referrals
Coordination of referrals to specialists and other healthcare professionals based on the patient's medical needs.
Preventive & Routine Care
Routine health assessments, medication monitoring, preventive care, and ongoing management designed to help patients maintain their health and independence at home.

First home visit
Once eligibility and coverage are confirmed, we schedule an initial comprehensive visit at home.
New Patients
New patients or their family members may contact Kifah Integrative Health to request an initial appointment.
Before the first visit, basic information will be collected regarding the patient's medical needs, insurance coverage, location, and eligibility for home-based medical care.
Once eligibility and coverage are confirmed, an initial comprehensive home visit can be scheduled.

Kifah Integrative Health accepts Medicare and participates with select insurance plans.
Insurance coverage and patient responsibility vary depending on the individual's health plan and benefits. Coverage will be verified prior to services whenever possible.
